Coffee May Cut Liver Cancer Risk

People who drink coffee regularly may face a substantially lower risk of liver cancer, cirrhosis and death from liver disease, according to a large study that followed nearly 355,000 adults for more than a decade.

The strongest association appeared among participants who reported drinking at least five cups a day. Compared with people who drank no coffee, they had a 47% lower risk of liver cancer, a 32% lower risk of cirrhosis and a 42% lower risk of dying from a liver-related cause.

The findings do not prove that coffee prevents cancer. The research was observational, meaning it identified a strong connection but could not establish direct cause and effect. Coffee drinkers may differ from non-drinkers in other ways that influence their health.

What the Study Found?


The research published in Clinical Gastroenterology and Hepatology examined 354,957 UK Biobank participants who did not have cirrhosis or liver cancer when the study began.

Participants reported how much coffee they drank, the type they usually chose and if they added sugar or artificial sweeteners. Researchers then used medical and death records to track new cases of cirrhosis, hepatocellular carcinoma and liver-related deaths over a median period of 13 years.

The largest reductions were found among people drinking five or more cups, but the pattern did not begin only at that level. Lower risks were also seen among people drinking one or two cups a day, with the association generally becoming stronger as consumption increased.

That point is important because the results should not be read as medical advice to begin drinking five large coffees every day. Cup size and caffeine content vary widely, and a café serving may contain considerably more caffeine than a small cup used in dietary research.

Decaf Coffee Was Linked to Similar Benefits

Researchers found similar associations for caffeinated and decaffeinated coffee. That suggests caffeine alone is unlikely to explain the results.

A review of the findings by Cedars-Sinai investigators points to other compounds found naturally in coffee, including chlorogenic acids, polyphenols and diterpenes.

Such compounds may reduce oxidative stress and inflammation. They may also affect the cells involved in fibrosis, the process through which repeated liver damage produces scar tissue.

The decaf finding also separates the new research from studies focused mainly on caffeine. We previously reported that the way caffeine remains in the bloodstream may influence body fat and diabetes risk. In the liver study, however, compounds that remain after decaffeination appear to be an important part of the explanation.

Liver Scans Supported the Main Results

The research went further than simply comparing coffee habits with diagnoses recorded years later. A subgroup of participants underwent liver MRI scans, while another group had thousands of proteins in their blood analyzed.

People who drank more coffee generally had lower levels of liver fat, iron, inflammation and fibrosis on imaging. Their blood protein patterns also pointed toward healthier liver function and less activity in pathways associated with scarring and disease progression.

Those findings do not establish that coffee caused the differences. They do, however, provide biological evidence that matches the lower rates of cirrhosis, cancer and liver-related death found in the wider group.

Earlier Research Has Found a Similar Pattern


The result does not stand alone. An earlier large UK Biobank study also found that coffee drinkers had a lower risk of developing chronic liver disease and dying from it.

Other reviews have linked coffee consumption with a lower risk of hepatocellular carcinoma, the most common form of primary liver cancer. The new study adds longer follow-up, detailed liver imaging and blood protein analysis to that existing evidence.

NCHStats recently examined another possible dietary connection involving protein metabolism and liver cancer development. That work involved experimental models and applied mainly to impaired liver function. The coffee findings come from a large human population, but they still cannot replace a controlled clinical trial.

Coffee Does Not Remove the Main Causes of Liver Cancer

Most liver cancer develops in people who already have chronic liver damage. The main recognized risk factors for liver cancer include chronic hepatitis B or C infection, cirrhosis, heavy alcohol use and serious metabolic liver disease.

Drinking coffee cannot cancel those risks. It cannot reverse advanced cirrhosis, replace hepatitis treatment or make heavy drinking safe.

Alcohol remains a major cause of liver damage in the United States. Our review of alcohol-related illnesses and deaths shows how excessive consumption contributes to liver disease, cancer and thousands of preventable deaths.

Maintaining a healthy weight, receiving appropriate hepatitis testing or treatment, limiting alcohol and managing diabetes remain more established ways to protect the liver.

Five Cups Can Be Too Much for Some People

The Food and Drug Administration says 400 milligrams of caffeine per day is not generally associated with harmful effects in most healthy adults. Five small cups may stay near that level, but five large mugs, strong cold brews or café drinks with extra espresso can easily exceed it.

Moderate coffee intake has also been linked to possible cardiovascular benefits. Our recent look at coffee consumption and heart health found that the most favorable results often appear among people drinking two to four cups per day. The evidence does not show that drinking more always produces greater protection.

Caffeine affects people differently. Some develop anxiety, shaking, a racing heartbeat, stomach discomfort or poor sleep after one or two strong drinks. Pregnant people and those with certain heart conditions, uncontrolled high blood pressure or medication interactions may need a lower limit based on medical advice.

Decaf remains a practical option for people who enjoy coffee but do not tolerate caffeine well. The liver study found similar associations for regular and decaffeinated coffee, suggesting that other natural compounds in coffee may provide much of the possible benefit.

People who already drink coffee can view the findings as another sign that moderate consumption may fit within a healthy diet. Plain or lightly prepared coffee is a better choice than drinks loaded with sugar, flavored syrup and cream.

People who dislike coffee do not need to begin drinking it solely to prevent liver cancer. The study found an association rather than a guaranteed protective effect, and an individual’s overall risk depends far more on liver disease, viral hepatitis, alcohol use, metabolic health and family medical history.

Across hundreds of thousands of adults, regular coffee consumption was consistently linked to healthier liver scans and fewer serious liver outcomes. Decaf showed a similar pattern, suggesting that the benefit may come from the coffee itself rather than the caffeine that keeps people awake.
